A HIQA inspection has revealed issues at an IPAS centre in Salthill.
The Eglinton Centre has 227 residents split between family rooms, townhouses and more than 40 bedrooms.
Although some issues were identified at the centre, in broader terms there was a lot of positivity.
The majority of adult residents spoken with agreed that services delivered were person-centred, and staff were helpful, respectful and kind.
Inspectors also found communal areas were clean, well-maintained and equipped, and residents were supported in personalising their spaces.
But there was significant room for improvement in many areas.
One apartment was affected by mould, while in another, a family didn't have enough bowls and plates to eat a meal together.
Some parents were required to share their bedroom with their children, and in some rooms children aged 10 or older were sharing bedrooms with siblings of the opposite gender.
Another risk identified was young children left unsupervised in their rooms and throughout the building for significant lengths of time, as well as using public transport unaccompanied.
Overall, inspectors held that a number of improvements were required in relation to governance, risk management, oversight systems, safeguarding and the assessment of resident’s needs.
