An unannounced inspection has revealed serious issues at a nursing home in Oranmore.
HIQA found that Brampton Care Home wasn't compliant with regulations in almost half the categories assessed.
On the day, feedback from residents was that staff were kind and caring - but a number weren't satisfied with the quality of the service overall.
A substanial number of residents told inspectors they often had to wait extended periods of time for help from staff.
One visitor told inspectors they rang the bell on behalf of a resident and no-one came.
Many residents said they found their days long in the centre and there was "nothing to do".
Inspectors noted during the visit that activities scheduled for day two were religious, including saying the rosary and reading scripture, with no alternative.
Some meal services were described as pleasant while others were found to be "chaotic and unsupervised".
Staff turnover was found to be high, and residents said this impacted on the service, as they had to keep familiarising themselves with new people, who weren't always aware of their needs.
The inspection also identified serious issues with governance and management of the centre, noting this is a repeat non-compliance.
